Job Overview:

• Job title: Registered Nurse (RN) - Home Health
• Location: New Lenox, IL
• Start Date: March 16, 2026
• Duration: 13 weeks with possible extension
• Shift: Days (8:00 AM – 5:30 PM, 40-hour guarantee)

Responsibilities:

• Provide patient care through the nursing process (assessment, planning, implementation, evaluation).
• Supervise CNAs and coordinate with other healthcare workers.
• Conduct comprehensive initial and follow-up assessments (physical, psychosocial, environmental).
• Initiate and update Plans of Care; establish discharge plans.
• Complete OASIS documentation.
• Educate patients, families, and caregivers.
• Ensure compliance with federal, state, and agency regulations.
• Promote safety, privacy, and confidentiality.
• Perform telehealth/virtual visits as needed.

• Home Health experience (required; strong Med Surg with some Home Health considered).
• Valid driver’s license & car insurance (required).
• Illinois RN license (active at submission).
• BLS certification (required).
• COVID & Flu vaccination (declinations not accepted).
• OASIS & Start of Care experience (strongly preferred).
• Local candidates prioritized.
