Day Habilitation Specialist

DDD Day Habilitation Program DirectorGrace Community Care & Homes is developing its Developmental Disabilities and Autism department and seeks a DDD Day Habilitation Program Director. The role will contribute to person-centered services, reliable operations and accurate documentation.Develop and oversee day-program operations, staffing, training, participant plans, safety, incidents, attendance and service documentation; coordinate with families and support coordinators.Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. This is a part-time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.
