Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Home health experience, reliable transportation, current driver's license, agency-required liability insurance Description: This is a Home Health Physical Therapist position. The contract assignment is for 13 weeks. The schedule is Monday to Friday, from 8:00 AM to 5:00 PM. The candidate will be visiting patients in their homes. There is potential for hire at the end of the contract. Candidates must be comfortable traveling up to 75 miles from their assigned care center. The average number of patients seen per day is 5-6. Weekly productivity expectations equate to 30 points. The consultant will utilize electronic medical records (EMR) systems. On-call duties may be required, one Saturday a month. Documentation must be completed within 24 hours of patient visits. The contractor must accept every patient on their schedule. Mileage expenses will be reimbursed at the IRS standard rate. Submissions from local candidates are accepted at the same rate. Candidates should not have current direct employment with any related locations.
