Minimum Requirements
Masters or Doctoral Degree in Physical Therapy from an accredited program. (Client requires Masters Degree or Doctorate).
Active Louisiana Physical Therapist state license required to start the assignment.
OASIS experience required. Prior home health (HH) experience required.
BLS Certification from AHA or ARC may be required.
Willingness to travel within a 50-mile coverage area. (Preferred: willingness to travel to all service areas).
Preferred: Skills or attributes demonstrating comfort working across dispersed patient homes and varied environments.

Assignment Details
Facility Type: Home Health.
Shift Name: Evenings.
Shift Details: Evenings 2000-0400.
Shift Schedule Provided by Client: M-F 8:00-16:30 (client-provided schedule; confirm with recruiter if different for this assignment).
Weekly Hour Guarantee: 40 hours.
On Call/Weekend Requirement: No.
Coverage Area: Up to 50 miles in all directions from base location.
Mileage Reimbursement: Yes, mileage between visits from first patient to last patient will be reimbursed.
