Clinic Case Manager Full Time Patient Care Professional Red Lodge, MT, US 7 days ago Requisition ID: 1078 Salary Range: $36.42 To $43.70 Hourly Status: Full Time – Non-Exempt Reports to: Clinic Manager __________________________________________________________________________________________ Purpose and Scope of Position This position is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. The Clinic Case Manager is focused on client centered services that link clients and other family members with health care, psychosocial and other home, and community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. Is responsible for assessment, planning, coordination, advocacy, education, monitoring and accurate, timely documentation in all program areas. This position is accountable for a designated patient caseload/population and plans effectively in order to meet patient needs. Is responsible for coordinating the care and services of identified patient populations for an extended period across the continuum of care for outpatient services and assisting with inpatient services as needed. __________________________________________________________________________________________ Job Requirements 1. Required Qualifications Graduate of an accredited school of nursing Current Montana RN License or current multi-state license with the ability to practice in all compact states. Current Healthcare Provider CPR certification or the ability to obtain within 90 days of hire 3 years of experience in a medical setting related to the above tasks, knowledge, skills and abilities or an equivalent combination of education and experience Or an equivalent combination of education and experience relating to the above tasks, knowledge, skills and abilities will be considered 2. Preferred Qualifications Experience working with Cerner is preferred 4. Special Requirements Must be able to reason independently and work with minimal guidance. Must be able to interpret data and make sound judgments based on those interpretations. Must deal effectively with a wide variety of personalities and situations requiring tact, judgment, and poise. Must be able to adapt to quickly changing priorities and schedules.
