Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• At least 12 months of home health experience within the past 3 years (client requirement).
• Experience with HCHB and OASIS documentation systems (client requirement).
• Ability to meet productivity expectations of 30 points per week and work a guaranteed 40 hours per week (client requirement).

Preferred Qualifications

• Previous experience working with complex patient caseloads in home health settings.
• Previous travel clinician experience preferred.

Assignment Details

• Shift Hours: Monday – Friday, 8:00 AM – 5:00 PM.
• Schedule: 5 days per week, 8-hour shifts.
• On-call: noted but not specified in detail.
• Must be able to meet productivity requirements as stated in minimum qualifications.
