Santa Rosa Community Health is seeking a Staff Physician to join the Patient Centered Medical Home Team in Santa Rosa, CA. Under Medical Director oversight, the physician delivers comprehensive medical services and collaborates with an interdisciplinary team.

Responsibilities include ongoing patient relationships, coordination of care, and participation in quality improvement initiatives. Board-certified or -eligible in family medicine with CA license and CPR is required; Spanish/English skills
