Physical Therapist

Role: Licensed Physical Therapist responsible for delivering personalized, one-on-one care to older adults in private homes and senior living communities. Responsibilities Conduct evaluations, develop individualized treatment plans, and administer evidence-based therapy sessions to enhance mobility, independence, and quality of life. Document patient progress accurately within the EMR system, ensuring compliance with regulatory standards. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual care modalities to extend reach and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility in applicable state(s). Degree from an accredited PT program. Proficiency with computers and EMR systems. Current CPR certification. Physical Requirements: Ability to perform moderate physical activities, including lifting, repositioning patients, and extensive walking.
