Job Title:  Physical Therapist - Outpatient Rehab Clinical CoordinatorDepartment: Outpatient TherapySupervisor: V.P. Community Services and Business Development Job Objective:This position provides clinical and operational leadership for Mozaic Senior Life's outpatient rehabilitation program while maintaining an active patient treatment caseload. Approximately 75–80% of time to direct patient care and 20–25% to coordination and leadership responsibilities. This a key role responsible for supporting high-quality, patient-centered therapy services that support wellness, recovery, functional independence, and aging in place as well as daily clinic operations, operational oversight staff coordination, program growth, and community engagement. Minimum Qualifications:Education: Bachelor's degree required; advanced degree preferredExperience: Minimum of 3–5 years of outpatient therapy and supervisory experience in geriatrics or neurological rehabilitation settingLicensure: Licensed (unrestricted) Physical Therapist (PT) in the State of Connecticut Key Competencies:Strong clinical, operational, and organizational skillsAbility to balance patient care responsibilities with leadership oversightStrong communication and relationship-building skillsCommitment to innovation, quality outcomes, and patient-centered careCollaborative leadership style aligned with Mozaic Senior Life's mission and values Essential Job Functions:Clinical LeadershipMaintain an active outpatient therapy caseloadLead by example through clinical excellence and patient engagementSupport evidence-based treatment approaches and quality outcomesPromote interdisciplinary collaboration across therapy disciplines Operational OversightAssist with therapist scheduling, workflow, and clinic operationsMonitor cancellations, no-shows, patient access, and scheduling efficiencySupport productivity and patient satisfaction initiativesAssist with operational problem-solving and workflow improvements Program Growth & InnovationAssist in developing new therapy and wellness programsSupport referral development and community outreach initiativesIdentify opportunities to expand services aligned with community needsCollaborate with leadership on strategic growth initiatives Community Engagement & PartnershipsBuild positive relationships with physicians, referral sources, and community partnersCollaborate with internal Mozaic programs to support integrated care pathwaysRepresent outpatient therapy services professionally within the community Regulatory & Quality OversightEnsure compliance with federal, state, payer, and organizational requirementsSupport audit readiness and documentation complianceParticipate in quality improvement and patient safety initiatives Team Leadership & DevelopmentProvide day-to-day support and guidance to therapy staffFoster a culture of accountability, teamwork, and innovationSupport onboarding, mentoring, and staff development efforts Other related duties as assigned. Physical and Mental Requirements:Ability to sit or stand for long periods of time (often 6-8 hours per day).Ability to lift, transfer, and reposition patients (typically up to 50 lbs. independently; more with assistance or equipment)Frequent walking, bending, stooping, kneeling, crouching, and reaching. Ability to perform manual therapy techniques requiring upper body strength and coordination as well as hand/eye coordination such as keyboard skills, writing or picking up paper.Ability to communicate clearly and compassionately while maintaining professionalism and empathy in emotionally demanding situations.Ability to motivate people who may be reluctant, fatigued, declining or who have cognitive issues.Ability to maintain focus with some interruptions while supervising clients and preventing falls and injuries during mobility tasks. Able to hear sounds within some background of noise as in answering the phone, etc.Visual acuity to see clients, a computer and read printed documents.Ability to use company software and equipment to accomplish electronic tasks.Ability to stay focused on tasks for more than 60 minutes at a time.Ability to understand and relate to the reasons behind several related concepts.Ability to remember multiple tasks /assignments given to self and others over long periods of time. Environmental Conditions:Rare exposure to blood, body tissue or fluids.Possible exposure to high/low temperatures. This job description is not designed to cover or contain every possible listing of activities, duties or responsibilities that are required of the employee for this job.
