ED Care Coordinator, Day Shift, OP Care Navigation

$34.58 - $55.76 per hour

White Oak Medical Center White Oak Medical Center seeks to hire an experienced ED Care Navigator for our OP Care Navigation department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing. As an ED Care Navigator, You Will Identify patients who are vulnerable, high risk for readmission within 30 days of discharge, and/or high ED utilizers requiring additional support and connections to community resources. Collaborate with patients, ED staff, ED physicians, and community providers to develop a comprehensive plan of care for safe discharge and complete required documentation in a timely and accurate manner. Act as a patient advocate by identifying and connecting patients to the most appropriate community resources, maintaining a directory of available services by geographical area, and updating others about new services. Consult with others to identify potential community resources for resolving client health, psychosocial, or financial barriers. Collaborate with behavioral health team and other community groups to ensure patients are connected to the most appropriate mental health programs in the community. Identify patients with admitting orders still in the ED. Interview and complete assessment on the patients/family members to formulate a plan of care prior to transferring to a unit. Collaborate with ED interdisciplinary team to create CRISP Care Alerts for complex and high-utilizing patients. Qualifications Empathic toward individuals’ complex health needs and their families. Strong communication and customer service skills. Knowledge of community resources. Strong writing skills. Strong organizational skills: able to develop workflows to create more efficient processes. Ability to work in a fast‑paced environment while managing multiple priorities and maintaining professionalism. Excellent time management skills. Ability to problem solve and exercise professional judgment. Graduate of an accredited school of nursing with a minimum of an associate of science degree required; a bachelor’s degree preferred. 1–3 years’ experience in an acute care setting; at least 1 year of prior experience as a case manager, discharge planner, or care coordinator desired. Maryland RN licensure, BLS. Work Schedule Employment Type: Full‑time Hours per week: 40 hours/week Typical Daily Schedule: 12‑hour shifts – Day Shift Shift Type: Rotating Day Shift – Weekends and Holidays Holiday Requirements: Follow company holiday calendar Pay Range $34.58 – $55.76 If the salary range is listed as $0 or if the position is per diem (with a fixed rate), salary discussions will take place during the screening process. Those testing positive are given the opportunity to re‑apply in 90 days, if they can truthfully attest that they have not used any nicotine products in the past ninety (90) days and successfully pass follow‑up testing.
