Schedule: Monday - Friday 8 AM - 5 PM | Split between two locations

That's why we're looking for a compassionate Behavioral Health Care Manager to help bridge the gap between physical and behavioral health services. In this role, you'll become a trusted partner for patients who need support navigating life's challenges while improving their health outcomes.

Working within the Collaborative Care Model (CoCM), you'll have the opportunity to make a meaningful difference in patients' lives by providing evidence-based interventions, coordinating care with medical providers and psychiatric consultants, and helping patients achieve healthier, more fulfilling lives.

You may be a great fit if you have:

• Master's-level licensure in a behavioral health field (LCMHC, LCSW, LMFT), or associate-level licensure (LCMHC-A, LCSW-A, LMFT-A)

• Experience with mental health screening, assessment, and treatment planning

• Strong communication and relationship-building skills

• Comfort working with patients both in person and by phone

• Familiarity with evidence-based behavioral health interventions and psychopharmacology

• Strong organizational and documentation skills

• A compassionate, approachable, and patient-centered mindset
