High Rock Internal Medicine
Job Description
Job Description
Location: Lexington, NC
Practice: High Rock Internal Medicine
Job Type: Full-Time | On-Site
Schedule: Monday - Friday 8 AM - 5 PM | Split between two locations
Every day, patients walk through our doors carrying more than physical health concerns. Many are struggling with anxiety, depression, substance use disorders, grief, stress, or other challenges that affect their overall well-being. Too often, behavioral health needs go untreated because access to care is limited.
At High Rock Internal Medicine, we believe mental health is healthcare.
That's why we're looking for a compassionate Behavioral Health Care Manager to help bridge the gap between physical and behavioral health services. In this role, you'll become a trusted partner for patients who need support navigating life's challenges while improving their health outcomes.
Working within the Collaborative Care Model (CoCM), you'll have the opportunity to make a meaningful difference in patients' lives by providing evidence-based interventions, coordinating care with medical providers and psychiatric consultants, and helping patients achieve healthier, more fulfilling lives.
This is more than a behavioral health position—it's an opportunity to be part of a team that recognizes the powerful connection between mind and body and is committed to caring for the whole person.
Why This Work Matters• Help patients access behavioral healthcare within a trusted primary care setting
• Reduce barriers to treatment for individuals who may not otherwise seek help
• Support patients managing chronic medical conditions impacted by behavioral health concerns
• Work collaboratively with physicians, advanced practice providers, and psychiatric specialists
• See the direct impact of your work through improved patient outcomes and quality of life
• Build long-term relationships with patients while guiding them toward sustainable wellness
Why You'll Be Successful in This RoleWe're looking for someone who combines clinical expertise with genuine compassion for others. The ideal candidate is passionate about helping people improve their overall well-being and enjoys working as part of a collaborative healthcare team.
You may be a great fit if you have:
• Master's-level licensure in a behavioral health field (LCMHC, LCSW, LMFT), or associate-level licensure (LCMHC-A, LCSW-A, LMFT-A)
• Experience with mental health screening, assessment, and treatment planning
• Strong communication and relationship-building skills
• Comfort working with patients both in person and by phone
• Familiarity with evidence-based behavioral health interventions and psychopharmacology
• Strong organizational and documentation skills
• A compassionate, approachable, and patient-centered mindset
Why You'll Love Working Here• Your work has purpose—you'll help transform lives every day
• You'll be part of a supportive, collaborative team that values your expertise
• Continued learning, mentorship, and professional growth opportunities
• Predictable weekday schedule with no weekends or on-call responsibilities
• Modern practice environment focused on integrated, whole-person care
• A culture built on compassion, respect, and excellence
Why Our Benefits MatterWe believe that caring for our team members is just as important as caring for our patients.
• Competitive compensation that recognizes the value and impact of your expertise
• Comprehensive health, dental, vision, life, and disability insurance coverage
• 401(k) retirement plan with employer support after one year of employment
• Generous paid time off and paid holidays
• Continuing education and professional development support
• Modern, collaborative workspace with onsite diagnostics and updated technology
• No weekends or on-call responsibilities, supporting a healthy work-life balance
• A values-driven culture rooted in integrity, kindness, and excellence

Employment decisions are made without regard to race, color, religion, sex, national origin, age, disability, genetic information, veteran status, sexual orientation, gender identity, or any other protected status under applicable law.
At High Rock Internal Medicine, we're not just filling a position—we're investing in someone who wants to make a lasting impact on the health of our community.
