Care Manager: LTSS Care Coordination & Outcomes

Molina Healthcare is seeking a care management professional to coordinate long-term services and supports (LTSS) for high-need members. You will work with a multidisciplinary team to ensure timely assessments, care planning, and seamless service delivery across settings. Responsibilities include waiver processes, care plan development, and monitoring outcomes with attention to psycho-social and medical barriers.
