About the Role We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Responsibilities Build strong relationships with the facility and maintain a patient-centered approach Conduct on-site visits and provide remote care to residents Document patient visits and maintain accurate records Collaborate with other healthcare professionals to provide comprehensive care Participate in Continuous Quality Improvement (CQI) initiatives to enhance patient outcomes Requirements Active NP or PA license in the state of practice 1+ year of experience in: Geriatric care Documentation review Excellent communication, collaboration, and teamwork skills Masters degree in Nursing or Medical Education What We Offer Competitive salary range of $20,000 - $22,000 ANNUALLY Hybrid role with flexible scheduling, including the option to work from home Support and training from our clinical directors, regional managers, and supervisors Free CEUs and referral bonuses Collaborative team environment that prioritizes your growth and well-being Work 10 hours per week, only 1-2 days a month at the facility, the rest from home Join Our Team At Carease Health - FL, we believe in teamwork, compassion, and supporting one another to make a real difference in patient care.
