Salary $24.88 - $37.32 Hourly $51,755 - $77,633 Annually This position is a Pay Band C6 Why does the job exist? Provide direct client services for medically and financially eligible clients to attain mutually determined goals; gather, generate land maintain appropriate records and accurate completion of forms for social work intervention, expand, enhance and contribute to the improvement of services of the Children's Medical Services land related information needed to assure accurate, competent, and professional implementation of the Children's Medical Services program and the Public Health Division Mission statement. How does it get done? To provide direct client services to clientele. Will provide services to multi-ethnic, mono-lingual population, and provide assurance of services for children and youth with special healthcare needs. Services will be addressed on an individual, family, and systems level. Who are the customers? The Public and Children's Medical Services Program participants. Ideal Candidate The Ideal Candidate will have experience with case management; good communication skills; and the ability to multi-task. Minimum Qualification Bachelor's degree in Social Work (in accordance with NMAC 61-31-9) and two (2) years of experience in a social work setting such as social service agencies, healthcare, medical, educational settings, mental health, home health, hospice, developmental disabilities, and other relevant experience, working with patients, clients, providers, and families. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience. Employment Requirements Must possess and maintain a valid Driver's License. Working Conditions Services are provided in communities, clinics, auxiliary site settings, and in the homes of consumers to a diverse racial and ethnic population, many of whom do not speak English. Reaching and lifting up to fifty (50) pounds may be required. Potential exposure to communicable diseases. Travel to provide outreach and coverage, to attend meetings and training sessions, with the occasional overnight stay is required.
