Job Summary:
Seeking a dedicated Travel Social Worker to support acute care patient discharge planning, behavioral health assessments, and substance use evaluations. This role requires strong communication skills and adaptability to various clinical environments.

Responsibilities:

• Coordinate patient discharges and create comprehensive discharge plans.
• Conduct behavioral health assessments, including substance use and GOC conversations.
• Perform SBIRT and other substance screening assessments efficiently.
• Collaborate with interdisciplinary teams to ensure optimal patient care and smooth transitions.
• Maintain accurate documentation using electronic medical records (Meditech, Midas, CarePort).
• Demonstrate professionalism, friendliness, and flexibility to adapt to different unit needs.

Qualifications:

• Current Colorado Social Work license required.
• Minimum of 1 year experience as a Social Worker, preferably in acute care settings.
• Proven skills in discharge planning, behavioral health, and ED experience preferred.
• Strong communication and interpersonal skills.
• Ability to adapt quickly and work effectively in fast-paced environments.
