Key Responsibilities\n- \n
- Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues, playing a crucial role as part of an integrated care team. \n
- Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, to provide detailed recommendations for treatment plans to primary care providers. \n
- Offer brief, focused interventions tailored to the unique needs of patients in need of mental health services. \n
- Advise primary care providers on patient referrals to specialized mental health facilities when necessary, ensuring that patients receive optimal care according to their conditions. \n
- Initiate follow-up procedures to monitor patient progress and make necessary adjustments to treatment plans based on efficacy. \n
- Develop relapse prevention strategies and support patients in maintaining stability in their mental health. \n
- Detect patients at risk of worsening conditions and collaborate on proactive plans to stabilize and improve their mental health. \n
- Coordinate care by linking patients with other treatment providers, both within the healthcare system and externally, with patient consent. \n
- Document patient progress and maintain accurate records within treatment charts to ensure comprehensive patient histories are accessible for future care. \n
- Keep primary care providers informed of patients' evolving needs and assist in the formulation of tailored treatment plans. \n
- Empower patients with self-management skills and educational resources to facilitate their involvement in their healing journey. \n
- Consult with clinic management and other team members to enhance behavioral services and identify areas for outcome and program evaluation. \n
- Support patients in adhering to any medical treatments initiated by primary care providers, including strategies for managing medication side effects. \n
- Uphold the highest ethical standards in clinical practice by adhering to the Code of Ethical Conduct for Clinical Professionals. \n
- Effectively undertake additional duties as assigned by management. \n
The Licensed Professional Counselor will engage with a diverse patient demographic, encompassing individuals from various backgrounds and presenting various mental health needs. The focus will be on fostering a safe, supportive environment where patients can address their mental health issues holistically, integrating therapeutic practices with primary healthcare services to provide comprehensive care.
Qualifications\n- \n
- Master's Degree in Clinical Counseling or a closely related field is required. \n
- Possession of a current LPC (Licensed Professional Counselor) License. \n
- Solid understanding of mental health assessment protocols, treatment planning, and developmental psychopathology. \n
- Proficiency with systems and techniques in family therapy, group therapy, child psychotherapy, DSM-IV diagnoses, and behavioral treatment methodologies. \n
- Bilingual English-Spanish speaking skills are preferred, enabling better support for the community we serve. \n
- Ability to demonstrate sound mental and physical health, aligning with the demands of the role. \n
- Knowledge of the personal and social impacts of mental and physical health issues on individuals and their families. \n
- Familiarity with community resources and their utilization in support of treatment plans. \n
- Expertise in psychodiagnostics and psychotherapy techniques. \n

- Prior experience in a telehealth setting is a plus, providing insight into virtual care delivery. \n
- Clinical experience with diverse populations is preferred. \n
- Demonstrated ability to work in an integrated care environment will be beneficial. \n
The position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
