City/State Suffolk, VA Work Shift First (Days) Hours: 8:00 - 4:30 PM Telephonic: Intermittent Calls in a six week period Overview Sentara Obici Hospital is hiring an Inpatient Case Manager, Inpatient Case Manager, Registered Nurse. Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only. BLS within 90 days of hire. BSN is preferred. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility. 1-year acute care case management experience preferred. The base pay range for full-time employment is $89,544.00 - $136,468.80 annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
