Responsibilities:

• Provide direct patient care, including assisting with activities of daily living, such as bathing, dressing, and grooming

• Monitor and report changes in residents' conditions to the nursing staff

• Assist with mealtime, including setting up trays and feeding residents

• Maintain a clean and safe environment, adhering to infection control protocols

• Document patient information accurately and thoroughly

• Collaborate with the interdisciplinary care team to develop and implement individualized care plans

• Participate in ongoing training and education to stay up-to-date on industry best practices and regulatory requirements
