Internal Medicine Hospitalist - Post-Acute Care

About the role

Gold is a physician-owned medical group delivering integrated medical management across post-acute settings — inpatient rehabilitation facilities (IRF), acute rehab units (ARU), skilled nursing (SNF), and long-term acute care (LTAC). What makes us different is the clinical model: we practice PM&R and internal medicine as a co-management team, not as separate specialties consulting each other reactively. Physiatry owns the rehabilitation trajectory; internal medicine owns medical complexity — both present from admission through discharge.

Most post-acute groups are physiatry-only or hospitalist-only. We built Gold around the integration because it produces better outcomes for the medically complex patients this setting actually sees.

You’ll serve as an internist embedded in the rehabilitation care team, managing the medical complexity that drives post-acute outcomes — diabetes, heart failure, COPD, renal disease, infections, polypharmacy, deconditioning — proactively, throughout the stay, rather than on reactive consult. You round alongside PM&R, share responsibility for discharge planning and CMS-compliant documentation. This is internal medicine with a genuine, owned clinical role — not consultation at the margins.

Responsibilities

• Manage active medical issues for rehabilitation patients from admission to discharge
• Co-manage patients with PM&R, coordinating care plans and attending coverage
• Participate in weekly IDT conferences and contribute to IPOC and discharge planning
• Document to support medical necessity and IRF level-of-care requirements
• Partner with facility teams and Gold clinical leadership on quality and outcomes

Qualifications

• BC/BE in Internal Medicine (or Family Medicine with strong inpatient/hospitalist experience)
• Active or obtainable state license in CO
• Hospitalist or post-acute experience preferred; strong candidates from acute-care hospitalist backgrounds who want a more integrated, longitudinal model are encouraged to apply
• Comfort managing multi-morbid patients and working collaboratively across specialties
• Interest in post-acute medicine as a clinical home, not a stopover

• 7-on/7off
• M-F
• Weekends (or alternating weekends)
