Provide physical therapy treatments to skilled nursing and long-term care patients under the supervision of a licensed Physical Therapist. Coordinate with the PT on patient identification, evaluations, and referrals; collaborate with care teams; maintain regulatory documentation; and ensure physician orders are current.
