We are seeking a compassionate and skilled Registered Nurse (RN) to join our team at Avir at Windcrest, a 120-bed premier skilled nursing, rehabilitation care center and certified Alzheimer facility in Abilene, Texas. As a RN with our team, you will play a vital role in providing high-quality, patient-centered care to our residents.

About the Role:

As a Registered Nurse (RN) at Avir at Windcrest, you will be responsible for:

• Providing direct patient care to our residents, including assessing, planning, implementing, and evaluating individualized care plans

• Developing and maintaining effective relationships with patients, families, and healthcare team members

• Collaborating with interdisciplinary teams to ensure seamless transitions and optimal care outcomes

• Participating in quality improvement initiatives to enhance patient satisfaction and clinical outcomes

• Maintaining accurate and thorough records of patient care and treatment

Responsibilities:

• Conduct initial and ongoing assessments of patients, identifying and addressing physical, emotional, and psychological needs

• Develop and implement individualized care plans, incorporating patient goals, preferences, and values

• Provide skilled nursing interventions, including wound care, medication management, and Vital Sign monitoring

• Collaborate with physical, occupational, and speech therapists to ensure comprehensive rehabilitation and post-acute care

• Educate patients, families, and caregivers on disease processes, treatment plans, and self-care techniques

• Participate in routine care conferences and interdisciplinary team meetings to share information and coordinate care
