About Family Health Centers Family Health Centers is a Community Health Center that delivers primary and preventive health care to underserved populations. Job Overview As part of the Behavioral Health Services Department, Licensed Clinical Social Workers (LCSWs) serve as BH consultants working collaboratively with the medical team and patients. LCSWs provide mental health, substance abuse, and health behavior change support in primary care, pediatrics, women’s health, and dental clinics. Key responsibilities include: Conducting behavioral health assessments Providing brief intermittent psychotherapy and behavioral medicine (relaxation training, weight management, health habits, sleep, hygiene, etc.) Offering crisis intervention and patient education Collaborating with medical providers to establish treatment plans Referring patients to community providers and services Enhancing patients’ social safety net through collaboration with behavioral health and social service staff Participating in program planning and development Delivering in‑service training to staff Job Requirements Master’s Degree in Social Work or equivalent. Licensed Clinical Social Worker license from the State of Kentucky or equivalent. Three years of clinical social work experience, including use of a personal computer with internet and word processing software.
