Job Summary: The Clinical Manager provides direct, day-to-day supervisory oversight of all clinical staff operating within the Jordan Valley Senior Care center and in the field (community/home settings), including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Medical Assistants (MAs). This position serves as the operational clinical leader responsible for maintaining standards of care, staff performance, regulatory compliance, and the smooth functioning of all nursing and clinical support services across the continuum of PACE care. The Clinical Manager also works in close collaboration with the Interdisciplinary Team (IDT), the Medical Director, and other PACE department managers to ensure participant-centered, high-quality care is delivered consistently in the PACE center, during home visits, during transportation, and at contracted care sites. This role carries dual accountability — for the people it supervises and for the clinical processes that govern their work. Key Responsibilities: Nursing Staff SupervisionProvide direct, daily supervisory oversight of all RN and LPN staff assigned to the PACE center and field operations, ensuring adherence to scope-of-practice requirements under Missouri Nurse Practice Act and organizational policies.Assign, coordinate, and monitor nursing staff scheduling to ensure adequate clinical coverage across all PACE center hours of operation and for home visit/field activities, accounting for participant acuity and census. Conduct regular one-on-one supervision meetings with nursing staff; provide timely, documented performance feedback and coaching.Lead, administer, or coordinate nursing performance evaluations (annual reviews and probationary reviews) in collaboration with the Center Manager.Review and approve nursing documentation in the electronic medical record (EMR) for accuracy, completeness, and timeliness; provide corrective feedback as needed.Monitor nursing staff compliance with mandatory training, continuing education, licensure renewal, and competency validation requirements; maintain current licensure records for all supervised staff.Participate in recruitment, interviewing, onboarding, and orientation of new nursing staff.Serve as a clinical resource and escalation point for nursing staff questions, complex participant situations, and urgent clinical concerns during the workday.Ensure MAs operate within their defined scope of practice and organizational policies; MAs do not administer medications unless operating under a specific state-authorized protocol with RN oversight.Assign MA daily tasks and workflow in coordination with the clinic schedule and IDT priorities. PACE Center Clinic OperationsOversee the day-to-day operational flow of the PACE clinic, including exam room readiness, equipment functionality, supply inventory, and scheduling coordination with the primary care and specialist providers.Ensure the clinic environment meets all applicable regulatory, safety, and infection control standards, including CMS PACE standards under 42 CFR Part 460 and Missouri DHSS requirements.Manage clinic supply and medication inventory processes, including coordination of medication orders, controlled substance accountability and supply reorder workflows.Oversee the preparation and maintenance of examination rooms and treatment areas; ensure equipment is calibrated, functional, and documented per maintenance schedules (e.g., AED, glucometers, oxygen equipment, nebulizers, wound care supplies).Ensure all clinical equipment inspection and maintenance logs are current and accessible for survey or audit review.Coordinate with the PACE center Manager and IDT regarding clinic appointment scheduling, participant flow, and urgent/same-day clinical needs.Oversee triage processes within the PACE center, ensuring nursing staff follow established triage protocols and escalation pathways for participants presenting with acute changes in condition.Ensure proper management of medical waste, sharps disposal, and biohazardous materials per Missouri DNR and OSHA regulations.Maintain oversight of specimen collection, labeling, and laboratory send-out procedures; ensure chain-of-custody documentation standards are met.Ensure all clinic-based clinical processes are documented, standardized, and reflected in current policy and procedure manuals. Field & Community Clinical OperationsOversee nursing staff conducting home visits, ensuring visits are completed per the IDT plan of care, documented timely in the EMR, and meet PACE standards for community-based care.Develop and maintain field nursing protocols, including safety procedures for home visit staff, communication expectations during field hours, and escalation procedures for urgent participant situations identified in the home.Monitor the quality and completeness of field nursing documentation, including home visit notes, medication reconciliation in the home, and identification of safety hazards in the participant's living environment.Ensure field staff conduct home safety assessments per PACE policy and document findings with follow-up actions for the IDT. Respond to or coordinate nursing response for urgent participant situations identified by field staff or transportation personnel during community activities.Ensure field staff adhere to organizational vehicle safety policies, participant transport protocols, and emergency procedures while in the community. Clinical Processes, Protocols & Standards of CareDevelop, implement, review, and update clinical nursing protocols and standard operating procedures (SOPs) annually or as warranted by regulatory change, quality findings, or incident review.Ensure clinical practice standards align with current evidence-based guidelines, CMS PACE regulations (42 CFR Part 460), Missouri Nurse Practice Act (RSMo Chapter 335), and organizational policies.Oversee and standardize clinical processes.Ensure nursing staff competency in all core clinical procedures through skills validation, return demonstration, and ongoing observation.Monitor clinical process metrics (e.g., medication error rates, fall rates, wound healing outcomes, missed assessment rates) and report trends to the Quality & Compliance Manager and Medical Director.Serve as the clinical escalation contact for urgent participant health concerns during PACE center hours and for after-hours nursing staff. Medication Management OversightOversee all aspects of medication management within the PACE center and field, ensuring compliance with CMS PACE medication requirements (42 CFR §460.98), and organizational medication management policies.Ensure all medication administration that occurs is by licensed staff working within their scope of practice.Oversee controlled substance receipt, storage, documentation, count, waste, and disposal per Missouri state law and DEA requirements; ensure accurate controlled substance logs are maintained and reconciled each shift.Monitor medication error reporting, conduct or facilitate root cause analysis of medication errors, and implement corrective actions; report trends to the Quality & Compliance Manager and Quality Committee.Coordinator with the PACE IDT members and other disciplines on medication protocols/management, medication reconciliation, and safety concerns.Conduct regular MAR audits and report findings. Interdisciplinary Team (IDT) Collaboration & Care CoordinationServe as a core IDT member, participating in all required IDT meetings and contributing the nursing perspective to participant care planning per 42 CFR §460.104.Ensure nursing assessments and updates are integrated into participant Plans of Care (POCs) and that POC goals reflect current clinical status, nursing interventions, and measurable outcomes.Coordinate nursing communication with IDT members regarding changes in participant clinical condition, new diagnoses, hospitalizations, and care transitions.Oversee nursing's role in care transitions management, including hospital discharge follow-up, SNF coordination, and post-acute monitoring protocols.Ensure timely nursing notification to the IDT and Medical Director of acute changes in participant condition.Collaborate with Social Work and the IDT on participant mental health, behavioral health, and psychosocial concerns that intersect with clinical care.Coordinate nursing support for participant end-of-life transitions, ensuring comfort care protocols are activated and family communication is facilitated in partnership with social work and the IDT. Participant Safety & Infection ControlServe as the primary clinical lead for infection prevention and control within the PACE center and field, in coordination with the Quality & Compliance Manager.Ensure compliance with CDC standard precautions, transmission-based precautions, and applicable Missouri DHSS infection control regulations.Oversee surveillance for healthcare-associated infections (HAIs), respiratory illness outbreaks, and communicable disease events among PACE participants.Manage outbreak response protocols for influenza, COVID-19, norovirus, scabies, or other communicable disease events affecting PACE participants or staff.Ensure all nursing and clinical staff complete annual infection control training and demonstrate compliance with hand hygiene, PPE use, and isolation protocols.Oversee immunization program for PACE participants (influenza, pneumococcal, COVID-19, shingles, Tdap) in coordination with the pharmacy and IDT; maintain immunization tracking records.Oversee OSHA bloodborne pathogen compliance, including staff training, sharps management, post-exposure protocols, and documentation. Emergency Preparedness & ResponseServe as a key member of the PACE center emergency preparedness team; develop and maintain nursing-specific emergency response protocols for PACE center emergencies (medical emergencies, fire, severe weather, power outage, and mass casualty events).Ensure all clinical staff are trained in emergency response procedures, including BLS/CPR, AED use, emergency medication administration, and evacuation protocols.Maintain current emergency contact lists, participant emergency evacuation registries (participants requiring assistance), and emergency supply inventories.Coordinate with the PACE Center Manager and leadership on participation in required emergency preparedness drills and annual emergency plan review per 42 CFR §460.72.Develop and maintain nursing-specific continuity of operations protocols for clinical services during declared emergencies or disaster events affecting PACE operations. Quality Improvement & Clinical AuditingParticipate actively in the PACE Quality Improvement Committee (QIC); contribute nursing and clinical operations performance data to QI activities.Design and conduct ongoing clinical audits to monitor nursing and clinical staff performanceNursing assessment completeness and timelinessReport clinical audit findings to the Medical Director and QIC with trend analysis and recommended corrective actions.Lead or participate in root cause analysis (RCA) for sentinel events, serious incidents, or patterns of clinical concern identified through auditing.Implement and monitor effectiveness of clinical corrective action plans (CAPs) arising from internal audits, external surveys, or QI initiatives.Support preparation for CMS PACE on-site program integrity audits and Missouri DHSS surveys by maintaining audit-ready clinical documentation and staff competency records. Regulatory Compliance — Clinical OperationsEnsure all nursing and clinical operations comply with applicable federal and Missouri state regulations.Ensure all nursing policies and procedures are reviewed annually, updated as needed, and reflect current regulatory requirements; coordinate policy sign-off with the Executive Director.Monitor compliance with CMS PACE participant-to-nursing-staff ratios and care standards; escalate concerns to the Center Manager immediately. Communication, Documentation & Information SystemsEnsure timely, accurate, and thorough nursing documentation in the PACE electronic medical record (EMR) system; establish documentation standards and audit compliance regularly.Maintain shift-to-shift communication systems, including clinical huddles, handoff protocols, and urgent communication escalation processes.Oversee the nursing on-call communication process, including after-hours triage coverage coordination and documentation of after-hours encounters.Ensure documentation of all reportable events (falls, incidents, medication errors, communicable disease events) is completed accurately and within required timeframes.Serve as the clinical operational liaison between clinical staff and the PACE center Manager. Leadership, Staff Development & Organizational CultureModel and reinforce a culture of clinical excellence, compassionate participant-centered care, accountability, and psychological safety for clinical staff.Lead clinical huddles to review participant census, acuity concerns, staffing assignments, and priority clinical issues for the day.Identify professional development needs among clinical staff and coordinate or facilitate access to continuing education, skills training, and career growth opportunities.Mentor newly licensed nurses and newly hired clinical staff through structured orientation and supervised practice periods.Facilitate resolution of interpersonal conflicts among clinical staff in a timely and constructive manner; escalate to Center Manager or HR as appropriate.Represent clinical staff perspectives in organizational planning, policy development, and interdisciplinary program design.Champion and implement initiatives related to participant safety, fall prevention, infection control, and clinical quality improvement across the clinical team.All other duties as assigned.
