Our client is seeking a dedicated Lead Primary Care Physician to join their growing team in the Homestead area of Pittsburgh. This full-time, in-person role is ideal for a physician passionate about providing high-quality care to patients with complex, chronic conditions and a strong commitment to serving Medicaid and Medicare populations. Who You Are

• 5+ years of clinical experience and administrative experience leading interdisciplinary teams.
• Experience working for a clinic, community-based organization & inpatient unit.
• Experience with training and mentoring.
• Excellent interpersonal communication skills.
• Strong problem solving, project management and organization skills.
• Independent self-starter and critical thinker interested in population health, healthcare utilization management, and the ongoing development of our care model.

Your Role

• Provide comprehensive primary care to a panel of approximately 250 patients, seeing 10 patients per day.

• Lead and collaborate with a multidisciplinary care team including case managers, behavioral health specialists, pharmacists, and nutritionists.

• Develop transitional care plans and coordinate discharges from hospitals, ERs, and skilled nursing facilities.

• Accept same-day/next-day urgent appointments to prevent hospitalizations.

• Utilize eClinical EMR for patient documentation and care coordination.

• Educate patients on chronic disease management and the importance of advanced directives.

How You Qualify

• MD or DO degree with Board Certification in Internal Medicine or Family Medicine.

• Active Pennsylvania medical license.

• BLS certification.

• Strong problem-solving skills and ability to engage patients resistant to treatment.

• Comfort working in a high-touch, value-based care model.
