Position Summary

As we continue to expand our Primary Care (PC) physician team, we are seeking a Healthcare Patient Outreach Assistant to help build patient volume and strengthen continuity of care. This role focuses on connecting with patients seen in our Urgent Care (UC) clinics who do not yet have a Primary Care Provider (PCP) within FMCOH.

You will be the friendly, upbeat voice that helps patients understand the value of establishing ongoing primary care - and you'll play a key role in improving patient outcomes and supporting our growing physician team.

Responsibilities

Essential Functions

• - Review daily lists of Urgent Care patients who do not have an FMCOH PCP.
• - Conduct 30+ outbound calls per day to recent UC patients (typically 1-2 days after their visit).
• - Engage patients in a warm, cheerful, and supportive manner to:
• - Review their recent UC visit.
• - Explain the importance of having a dedicated PCP.
• - Offer to help them establish care with an FMCOH Primary Care physician.
• - Schedule new patient appointments and coordinate next steps.
• - Document all outreach attempts and patient interactions accurately in the EMR.
• - Collaborate with the Primary Care team to support panel growth and patient continuity.
• - Maintain a positive, energetic, and patient-centered approach throughout all interactions.

Qualifications

Education/Training

• High School graduate or equivalent.

Licensure/Certification

• None.

Experience

• Six (6) months' experience in a clerical office, or general business, and/or customer service setting
