This position will work primarily days in UConn Health Pharmacy Services Inc. located in the Exchange, 270 Farmington Avenue, Suite 108 in Farmington, CT.SUPERVISION RECEIVED:Works under the general supervision of an employee of higher grade.SUPERVISION EXERCISED:May lead/supervise Pharmacy Technicians, Pharmacy residents/students and other employees of lower rank as assigned.EXAMPLES OF DUTIES:Provides prescription services to inpatient/outpatientChecks unit dose medication for appropriate drug, dose quantity and packaging integrityFacilitates availability of first doses and specialty itemsMaintains adequate medication suppliesCompletes monthly controlled substances audits and quality assessment roundsMaintains and enforces the documentation and security of narcotic suppliesChecks medication orders for therapeutic appropriatenessChecks transcription of medication ordersUpdates medications on administrative recordsProvides pharmacokinetic monitoring of patients receiving specific drugs and ensures that serum levels are drawnMonitors parental nutritionResponds to emergency codesObserves for adverse drug reactionsDocuments all medication incident reportsResponds to stroke alertProvides medication counseling to patientsProvides drug information to physicians, nurses, patients and other health care professionalsDocuments interventions with health care personnel related to drug therapyOversees deliveries to patient care floorsCompounds IV medications, including chemotherapy medsPrepares hyper alimentation solutionsDispenses/provides information on investigational drugsPerforms drug utilization evaluations on medicationsAssists in narcotic discrepancy investigation and resolutionPerforms quality assurance rounds and audits as assignedParticipate in patient care rounds, attend departmental staff meetings, educational seminarsContributes to pharmacist continuing education seminarsPreceptors' hospital pharmacy residentsProvides in-service education to hospital health care professionalsTroubleshoots pharmacy technology related issuesMay be clinical instructor for university undergraduate clinical pharmacy externship and pharmacy experience courseLectures students in undergraduate programMaintains patient confidentialityPerforms related duties as requiredMINIMUM QUALIFICATIONS REQUIREDKNOWLEDGE, SKILL AND ABILITY:Knowledge of the principles and practices of pharmacy and pharmacology and their application to the operation of hospital pharmacy, long-term care pharmacy, home-infusion pharmacy, specialty pharmacy, and outpatient clinicsKnowledge of pharmaceutical waste managementKnowledge of relevant Federal and State laws and regulations, including USP 797 and USP 800Considerable interpersonal, oral and written communication skillsAbility to work with technical automated equipment and information systemsAbility to precept studentsAbility to maintain recordsTeaching abilityEXPERIENCE AND TRAININGGeneral Experience:PharmD or bachelor's degree in pharmacy from an accredited Pharmacy program.Must possess and maintain license as a Registered Pharmacist in the State of Connecticut.PREEFERRED KNOWLEDGE/ EXPERIENCE:Two (2) years retail/community and specialty pharmacy experienceOne (1) year home infusion pharmacy experienceExperience in pharmacy software applications, automated medication system, EPIC, CPR+, and QS1Knowledge of 340b contract pharmacy operations and distribution practices including cold chain and vaccine handlingWORKING CONDITIONS:May be exposed to hazardous materials, infectious/communicable diseases and some disagreeable conditions.SCHEDULE: Full time, 80 hours every two weeks, primarily 9:30 am to 6:00 pm, schedule includes evenings, weekends and holidays as assigned, on-call responsibilities for after-hours coverage as assigned, 30-minute unpaid meal break.Why UConn HealthUConn Health is a vibrant, integrated academic medical center that is entering an era of unprecedented growth in all three areas of its mission: academics, research, and clinical care.
