About The Role We are seeking a dedicated Provider (Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant) to deliver comprehensive primary care to patients in their homes and community-based facilities, such as assisted living or skilled nursing facilities. About The Role We are seeking a dedicated Provider (Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant) to deliver comprehensive primary care to patients in their homes and community-based facilities, such as assisted living or skilled nursing facilities. Responsibilities Conduct in-home visits (minimum 25 per week), including assessments, exams, and medication reviews. Manage chronic conditions and develop individualized treatment plans. Serve as the primary care provider and conduct regular patient follow-ups. Educate patients on disease management and healthy living. Collaborate with interdisciplinary teams to ensure coordinated, high-quality care. Maintain accurate EMR documentation and comply with all healthcare and HIPAA regulations. Required Qualifications Active, unrestricted current licensure as a Provider (NP or PA) in the state of practice, clear of any clinical or board restrictions. Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Physician Assistant (PA). Ability to obtain autonomous provider status within six months of meeting eligibility requirements. DEA license and BLS certification. Valid driver's license, reliable transportation, and a clean driving record (MVR). High-speed internet service at home Ability to pass a comprehensive background check and 10-panel drug screen. 1+ year of experience in primary care, family practice, general practice, or outpatient care. Experience with: Risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA). Electronic Medical Records (EMR) documentation. Independent clinical decision-making and in-home assessments. Schedule Requirements Ability to maintain a highly mobile schedule traveling regularly to patient homes and provider facilities.
