This Per Diem / PRN Physical Therapist role provides the flexibility of mobile therapy and home-based care , allowing you to work independently while making a meaningful impact on patients in your local territory. Your Role: Home Health Physical Therapist As a Home Health Physical Therapist (PT) , you will deliver skilled physical therapy services in patients' homes , assisted living facilities, and senior communities throughout Los Angeles and surrounding areas . Your work will focus on rehabilitation, fall prevention, mobility improvement, and restoring independence for patients recovering from injury, illness, surgery, or chronic conditions including dementia and geriatric health challenges .
