RN/LPN – Nursing Home | Day Shift

Schedule: 7:00 AM–3:30 PM

Position Summary:
The RN/LPN provides compassionate, safe, and professional nursing care to residents in a skilled nursing and long-term care setting. The nurse is responsible for monitoring residents, administering medications and treatments, coordinating care, completing documentation, and communicating changes in residents' conditions to the appropriate healthcare providers and family members.

Essential Duties and Responsibilities:

• Provide direct nursing care to residents throughout the 8:00 AM–4:30 PM shift.

• Complete nursing assessments and monitor residents for changes in condition.

• Administer medications and treatments as ordered and according to facility policies.

• Monitor and document vital signs, blood glucose, weight, intake/output, and other clinical information as required.

• Provide wound care and other treatments within the nurse's scope of practice.

• Assist with resident care, comfort, hygiene, nutrition, hydration, and activities of daily living as needed.

• Review and follow each resident's care plan.

• Communicate changes in residents' condition to the physician, nurse practitioner, RN supervisor, Director of Nursing, and/or responsible party as appropriate.

• Coordinate with Certified Nursing Assistants (CNAs) to ensure residents' needs are met.

• Document all nursing care, medications, treatments, incidents, and changes in condition accurately and timely.

• Participate in physician visits, care conferences, and interdisciplinary team meetings as required.

• Monitor residents for falls, infections, skin breakdown, pain, behavioral changes, and other potential health concerns.

• Follow infection-control procedures, medication-management standards, HIPAA requirements, and facility policies.

• Provide compassionate support to residents and maintain their dignity, privacy, and independence.

• Communicate effectively with residents, families, CNAs, other nurses, physicians, and healthcare professionals.

• Complete shift-to-shift report and ensure continuity of care.

• Respond appropriately to emergencies and obtain additional medical assistance when necessary.

Qualifications:

• Current, unrestricted RN or LPN license in the state of employment.

• Previous long-term care or skilled nursing experience preferred.

• Current CPR certification, if required by the facility.

• Strong communication, organization, and documentation skills.

• Ability to work effectively with residents, families, CNAs, nurses, and other healthcare professionals.
