Registered Nurse - Home Health

Job Summary:

Provide comprehensive home health nursing care as a Care Manager RN, coordinating patient needs and ensuring quality outcomes. This role involves managing patient care plans and delivering skilled nursing services in patients' homes during day shifts.

Responsibilities:

Assess and develop individualized care plans for patients in a home health setting.
Coordinate care with doctors, patients, and family members to ensure optimal health outcomes.
Administer treatments, medications, and perform skilled nursing procedures as per plan.
Document patient progress and communicate updates to relevant healthcare teams.
Comply with all state and agency regulations, including maintaining current licensure and certifications.

Qualifications:

Possess a valid Registered Nurse (RN) license in the state of practice.
Have a minimum of 2 years of recent experience in home health or relevant clinical setting within the last 3 months.
Hold current BLS certification.
Possess a valid driver's license, automobile insurance, and reliable vehicle, and be at least 21 years old.
Provide RTO, birthdate, social security number, and schedule availability.
