About the Role:

We are seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our healthcare team. As an LPN (Licensed Practical Nurse) , you will be responsible for providing direct patient care to our residents, including administering medications, performing wound care, and collaborating with other healthcare professionals to ensure the best possible outcomes. Responsibilities:

• Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities

• Conduct initial patient assessments and develop individualized care plans

• Collaborate with other healthcare professionals to ensure coordination of care and effective communication

• Monitor and report changes in patient condition to the healthcare team

• Participate in quality improvement initiatives to ensure high-quality patient care

• Maintain accurate and thorough records of patient care and treatment

Requirements:

• Current Licensed Practical Nurse (LPN) licensure in the state of Missouri

• Experience as an Licensed Practical Nurse (LPN) in a geriatric setting or related field preferred
