Coverage Area: Lackawanna, Luzerne, and Wyoming Counties, PA

Key Responsibilities

• Provide skilled nursing visits in accordance with physician orders and individualized treatment plans

• Monitor patient conditions and report significant changes to the clinical team and physicians

• Maintain accurate and timely clinical documentation and medical records

• Educate patients and caregivers regarding treatment plans, medications, and ongoing care needs

• Coordinate care with physicians, clinicians, and interdisciplinary team members

• Support continuity of care through effective communication and follow-up

• Travel throughout assigned coverage areas to deliver patient care services

• Participate in care planning and team collaboration activities

• Ensure compliance with organizational policies and regulatory standards

Required Qualifications

• Current Licensed Practical Nurse (LPN/LVN) license in the state of Pennsylvania

• Graduate of an accredited practical or vocational nursing program

• Minimum one year of clinical nursing experience required

• Minimum one year of home health experience preferred

• Current CPR certification required while employed in a clinical role

• Valid driver’s license and reliable transportation required

• Strong clinical assessment, documentation, and communication skills

• Ability to work independently in a field-based environment

Pay Details: $55,000.00 to $62,000.00 per year
