Licensed Practical Nurse | PRN | | Nashville, TN

All Heart Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our home health team in the Nashville area. This PRN position offers flexible, as-needed scheduling while providing personalized, high-quality care to patients in their homes.

Key Responsibilities:

• Provide skilled nursing care to patients in accordance with the plan of care and physician orders
• Monitor patient health status and report changes to the supervising Registered Nurse or physician
• Administer medications and treatments as prescribed
• Assist with wound care, vital signs monitoring, and other clinical tasks
• Educate patients and caregivers on disease management, medications, and home care techniques
• Maintain accurate and timely documentation in compliance with company policies and regulatory standards
• Collaborate with the interdisciplinary care team to ensure coordinated patient care
• Travel to patient homes within the assigned service area

Qualifications:

• Current Licensed Practical Nurse (LPN) license in the state of Tennessee
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical and communication skills
• Ability to work independently and manage time effectively
• Previous home health experience preferred but not required

Work Schedule:

• PRN (as needed)
• Flexible scheduling based on patient needs and coverage requirements
