Job Summary:

Responsibilities:

• 
  Assess patient function through formal and informal testing to determine skeletal, neurological, respiratory, and cardiovascular status.
  
  
• 
  Develop goal-specific treatment plans based on assessment data and patient tolerance.
  
  
• 
  Submit and update treatment regimes, coordinate with physicians, and co-sign orders as necessary.
  
  
• 
  Design a treatment schedule tailored to patient needs, coordinating with clinical support staff and caregivers.
  
  
• 
  Educate patients, families, and caregivers on pathology, treatment options, prognosis, and intervention rationales.
  
  

Qualifications:

• 
  Possess active Michigan Physical Therapist license.
  
  
• 
  Hold current BLS certification from the American Heart Association.
  
  
• 
  Demonstrate a minimum of one year of pertinent outpatient physical therapy experience.
  
  
• 
  Provide current skills checklist relevant to physical therapy.
  
  
• 
  Submit two supervisory reference forms completed within the past two years.
