Work Environment\n

This role is primarily remote, with responsibilities centered around virtual consultations and occasional onsite visits at the Community Center. You will work a full-time schedule from Monday to Friday, between the hours of 8:00 AM to 4:30 PM, with occasional flexibility for evening or weekend hours as needed based on client requirements. We prioritize work-life balance, offering the ability to adjust your schedule to accommodate ongoing client needs. This position also includes on-call responsibilities to address urgent situations as they arise.

Key Responsibilities\n

As a Licensed Clinical Social Worker, your role includes:

• Conducting comprehensive psychosocial assessments for seniors to identify their emotional, mental, and social needs, and informing personalized care plans.

• Collaborating closely with seniors and their families to develop tailored care approaches that reflect individual preferences and quality of life goals.
\n
• Providing emotional support through counseling sessions, assisting seniors in navigating transitions such as loss, health changes, or relocation.
\n
• Advocating for client's rights in accessing community resources, services, and benefits to ensure they receive adequate support.
\n
• Engaging in interdisciplinary teamwork with healthcare professionals, including nurses and caregivers, to ensure seamless coordination of care.

• Making regular visits to client homes (as applicable) to monitor well-being and evaluate living conditions, ensuring safety and quality support.
\n
• Facilitating open communication among family members to resolve conflicts related to the care of the senior client.
\n
• Educating clients and families on community resources, support groups, and programs available to enhance their social engagement and overall quality of life.

• Maintaining accurate and up-to-date documentation in compliance with ethical standards using electronic health record (EHR) systems.

• Staying informed on best practices, emerging trends in social work, and relevant policies to ensure high-quality service delivery.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
