Pharmacy Technician Medicine Reconciliation

Hourly Pay Range: $20.99 - $32.53 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: Pharmacy Technician Med Reconciliation Location: Elmhurst Hospital Full Time: 40 hours/week Hours: Rotating Evening Shift (11am - 9:30pm, 12pm - 10:30pm); Monday-Friday; 10 Hour Shifts; 4 Shifts each week with rotating 3rd weekend and rotating holidays. What you will do: Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors.
