Job Title: Therapist (Part time)
Job Summary: The Therapist (LCSW/MFT/LPCC) provides direct counseling services to patients/clients. This includes assessments, counseling (individual, families, couples, group), treatment plans and crisis intervention.


Essential Duties and Responsibilities

• Conducts comprehensive assessments, treatment planning and scheduling.


• Ensures care coordination within the organization, the family unit, and the community.


• Maintains adherence to Hospital/Clinic policies and procedures.


• Maintains accurate, timely progress notes in participant medical records.


• Provides counseling sessions individually, family, couples or groups.


• Collaborates with Primary Care Providers


• Refers participants and families to appropriate community services.


• Complies with safety policies and procedures.


• Promotes and demonstrates respect for patient rights and confidentiality.


• Comfortable working with individuals with complex behavioral health presentations and/or substance use disorders.


• Has passion for helping individuals navigate a complex system of care.


• Demonstrates a willingness to be flexible and adaptable in all aspects of this work.


• Track referrals and provide follow-up to ensure service delivery


• Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.


• This position requires flexibility with scheduling and collaborating with other Behavioral Health Staff.


Qualifications:


• Master's Degree from an accredited school of Social Work, Psychology, or Licensed Professional Clinical Counseling.


• Five (5) years of counseling experience in a therapeutic setting.


• Experience in the health care field is advantageous.


• Minimum of one (1) year of experience working with a frail or elderly population preferred.


Licenses or Certificates:


• Current CPR and First Aid certification within six (6) months of hire.


• Valid California Driver's License.


• Licensing by the California Board of Behavioral Sciences, as a licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Clinical Counselor or Licensed Educational Psychologist.


Job Skills:


• Ability to work effectively in a team environment.


• Age-specific competency in working with the elderly.


• Strong knowledge of community services for the elderly and their families.


• Excellent interpersonal and communication skills.


• Adaptability to change of priorities and workload.


• Proficiency in computers and medical systems.
