We’re seeking a dedicated, full-time Hospice Registered Nurse Case Manager to walk alongside patients and families with skill and grace. This role has competitive pay ($70,000–$88,000 BOE) and a generous benefits package. Employer paid Life and Short-term Disability Bereavement leave that allows for support of you and your family 403(b) plan with match Hospice Registered Nurse Case Manager’s Responsibilities: (Included, but not limited to) Care for patients where they call home. Use your clinical skills and emotional support skills to support patients and families. Create care plans that treat the whole person—body, mind, and spirit. Work closely with providers, referral sources, and our amazing interdisciplinary team (IDT). Be a steady, calming presence during a difficult time—and help others feel just a little more at peace. Educate families, empower caregivers, and be the go-to person when people have questions.
