Responsibilities Provide emotional, social, and psychological support to residents, addressing their physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with residents, families, and healthcare professionals. Coordinate and facilitate resident outings, social events, and activities, ensuring a sense of community and social engagement. Maintain accurate and detailed records of resident care, progress, and interactions. Communicate effectively with residents, families, and staff members to ensure a seamless care experience. Collaborate with interdisciplinary teams to address resident needs and concerns. Participate in resident assessments, care planning, and care delivery. Requirements High school diploma or equivalent required. Degree in social work, psychology, or related field preferred. 1-2 years of experience in social services or related field; passion for working with seniors a must. Strong communication, interpersonal, and problem-solving skills. Ability to maintain confidentiality and discretion when handling sensitive information.
