Estella Byrd Whitman Community Health Center is seeking an experienced and compassionate Advanced Practice Registered Nurse (APRN/FNP) to join our primary care team.

We are looking for a provider who is comfortable caring for patients across the lifespan and who understands that good primary care is about more than treating the problem in front of you. Our providers play an important role in helping patients manage chronic conditions, stay current with preventive care, navigate referrals, and establish a consistent medical home.

As a community health center serving patients who may face financial, transportation, insurance, and other barriers to care, we are looking for someone who is clinically strong, dependable, patient-centered, and comfortable working as part of a small, collaborative team.

Responsibilities

• Provide comprehensive primary and preventive care to established and new patients.
• Perform health histories, physical examinations, assessments, diagnoses, and treatment planning.
• Manage common acute conditions and chronic illnesses, including hypertension, diabetes, hyperlipidemia, and other primary care conditions.
• Prescribe medications and order laboratory testing, diagnostic studies, referrals, and other appropriate services within the provider's scope of practice.
• Review and follow up on laboratory results, diagnostic testing, referrals, and other outstanding clinical needs.
• Identify and address preventive health and chronic disease care gaps.
• Provide patient education regarding medications, diagnoses, lifestyle changes, preventive care, and treatment plans.
• Maintain timely, accurate, and complete documentation in the electronic health record.
• Work collaboratively with the Medical Director, nursing staff, medical assistants, referral staff, population health staff, and other members of the care team.
• Participate in quality-improvement and population-health activities.
• Follow EBWCHC clinical policies, HIPAA requirements, infection-control standards, and applicable federal and state requirements.
• Participate in call coverage as assigned.

Qualifications

• Current Florida APRN license in good standing.
• Family Nurse Practitioner certification preferred.
• Current prescriptive authority as required by Florida law.
• Current DEA registration, if applicable to the provider's scope of practice.
• Current BLS/CPR certification.
• Experience providing outpatient primary care preferred.
• Comfortable caring for adult and geriatric patients; experience with pediatric patients is a plus.
• Strong clinical judgment and ability to work independently while collaborating appropriately with the healthcare team.
• Strong communication, documentation, and follow-through skills.
• Experience with Athenahealth or eClinicalWorks is a plus.
• Experience in an FQHC, community health center, rural health clinic, or other safety-net setting is preferred but not required.
