1 year Certified Home Care experience.
1 year Case Management & nbsp; Experience. Hospice,
private care, or working as a Case Manager on its own does not meet the
qualifications for Certified Home Health Care Management experience.

specifically within a certified home health agency will be considered relevant
for the 1-year requirement & nbsp

Ability to travel to cases as assigned. Weekend rotations are once every 6 weeks.
