Hospice RN Case Manager: Compassionate Home Care Leader
Legacy Hospice, Inc.
Legacy Hospice is seeking a Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care to patients in their homes in Demopolis, AL. You will coordinate care with an interdisciplinary team and support families with empathy and respect. The role requires an active RN license, 1 year hospice/home health experience, and a willingness to participate in on-call rotations.
