Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments and develop tailored treatment strategies
• Implement physical therapy procedures focusing on mobility, strength, balance, and functionality
• Collaborate effectively with nurses, doctors, case managers, and rehabilitation professionals to achieve optimal outcomes
• Provide education to patients and families regarding therapy objectives, discharge processes, and at-home exercises
• Document evaluations, treatment sessions, and patient advancements in alignment with facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure continued coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous, yet not required

Skills

• Substantial prior experience is highly regarded
