RN Care Case Manager

Cheer Home Care is looking for an RN Care Case Manager who loves spending time with older adults and providing high-quality care in the home. The RN Care Case Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides ongoing feedback to clients, client representatives, caregivers, and nurses. This role works closely with our office team and requires excellent communication and problem-solving skills.

This position serves as the primary care manager, coordinating care for clients among caregivers, RNs, therapists, and outside representatives. It requires strong clinical expertise and experience working with private pay clients and their families.

Schedule flexibility available: option to work one weekend day and have one weekday off.

Requirements:

• Upbeat personality
• Active California RN License, required
• Minimum 23 years of home health nursing experience, required
• Experience managing private pay clients, required
• Master's in Nursing, preferred
• Home care/home health industry experience, preferred
• Strong computer skills
• Ability to multitask, drive, and take action
• Strong organizational and communication skills
• Pass Live Scan fingerprint clearance
• Valid driver's license and proof of insurance, required
• Cell phone with internet access
• Proof of negative TB results
• Willing to work one weekend a month as back-up on call

Compensation: $120,000+ based on experience, with potential for team bonus.
