RN - Hospice & Palliative Care Nurse

We are seeking a compassionate and dedicated Registered Nurse specializing in hospice care to join a supportive healthcare team focused on delivering exceptional end-of-life services. This role involves providing patient-centered care either in home settings or hospice facilities, ensuring comfort, dignity, and quality of life for patients and their families throughout the hospice journey.

Key Responsibilities

• Perform comprehensive assessments addressing physical, emotional, and spiritual needs of patients.
• Conduct initial physical evaluations and develop individualized care plans in collaboration with a multidisciplinary healthcare team.
• Administer medications and manage symptoms following established protocols.
• Educate and support patients and families on end-of-life care options and resources.
• Coordinate services to facilitate smooth hospice care delivery.
• Maintain accurate documentation of patient care activities and communicate changes in condition to the interdisciplinary group.
• Perform venipuncture or collect lab specimens as ordered.
• Notify pharmacy and patients regarding medications and supplies related to hospice care.
• Keep primary and consulting providers informed about patient status.

Education and Certification

Graduation from an accredited nursing program and current licensure as a Registered Nurse eligible to practice in the relevant U.S. region. Completion of hospice philosophy and care training is required. Current Basic Life Support (BLS) certification by the American Heart Association standards is mandatory.

Experience and Skills

Previous experience in hospice, palliative, or critical care settings is preferred. Strong communication skills, both oral and written, are essential for effective collaboration and patient-family education. Basic computer proficiency and the ability to perform simple mathematical calculations are necessary. Completion of a Mandatory Reporter course within six months of hire is required.

Work Schedule

This is a part-time position with evening and night shifts available, including PRN opportunities.
