Bilingual Health Plan Navigator & Wellness Coach
Trinity Health
Trinity Health is seeking a Full-time Health Educator to advise and counsel health plan members, patients, and staff on wellness and chronic disease management. The role involves motivational interviewing, group seminars, and coordination with healthcare teams to close gaps in care.
The candidate should possess an LVN license in California or a relevant Bachelor’s degree, with bilingual English/Spanish abilities and strong data/reporting skills.
