Primary Care Provider - Detox Center

Be the Healing Connection at Hooper Detox Center Where clinical excellence meets compassion for the most vulnerable. Central City Concern is seeking a dedicated Primary Care Provider with a passion for addiction medicine and serving people in crisis. Join a collaborative, multidisciplinary team at Hooper Detox Center , where every decision and every interaction supports individuals taking their first steps toward recovery. This is more than just a clinical role - it's a frontline opportunity to provide transformative care to people experiencing homelessness, substance use disorders, and serious mental illness. You'll work alongside our experienced Medical Director to deliver evidence-based withdrawal management, with a focus on trauma-informed care, dignity, and connection. Why this role matters At Hooper, patients arrive in crisis. You'll use your clinical skills from performing physical exams to initiating Buprenorphine - help stabilize their health. But it's your compassion, cultural humility, and deep understanding of marginalized communities that builds trust and opens the door to long-term recovery. In this role, you will: Conduct physical exams and oversee withdrawal management protocols Provide ongoing care and crisis response in a fast-paced detox setting Collaborate with a compassionate team of nurses, case managers, and behavioral health providers Help shape recovery pathways for some of our community's most vulnerable members Make a lasting impact - one life at a time. Location: Hooper Detox Center (1535 N Williams Portland, OR 97227) Schedule: Morning shift 8am - 12pm, Afternoon shift 1pm-5pm This position can be a .6 fte (24 hrs) - 1.0 fte (40 hrs) a week position pending the Provider's licensure/certifications and experience. Salary Ranges (Full-Time) starting at:

NP/PA

$135,000 annually PA $135,000 annually Naturopath $142,000 annually Physician (MD/DO) $231,000 annually Minimum Qualifications NP/PA Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner applicants. If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and has unrestricted licensure to practice medicine in Oregon or eligibility for such licensure. Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or Licensed to practice as a Physician's Assistant in Oregon, with at least 2 years' experience.

MD/DO/ND

Board eligibility or certification in Internal Medicine or Family Practice. Graduation from a CNME-accredited, four-year Naturopathic Medical program. Must have completed a CNME-certified primary care residency program. Licensed naturopathic physician in the State of Oregon. Maintain an Active DEA Certification. All Applicants A strong background in mental health and addiction medicine is highly preferred, as well as experience working with unhoused and underserved/marginalized populations. Must have full prescriptive powers in Oregon with a DEA number. Demonstrated skills in primary care medicine with minimum of 2 years' experience preferred. Must have current BLS certification. Available to work during standard Hooper Center hours. Must meet CCC privileging requirements as required by FTCA. The OAR prohibits A&D program staff from having, within the previous 3 years, a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act. Must pass a pre-employment drug screen, TB test, and criminal background check. This includes clearance by the DHS Background Check Unit. Physical ability to bend, stoop, kneel, squat, twist, reach, pull, and lift moderately heavy objects. Able to climb stairs several times a day. Must adhere to agency non-discrimination policies. Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, lifestyles and sexual orientation, and treat individuals with respect and dignity. Essential Duties and Responsibilities Familiarity with site specific standing orders, workflows, communication standards. Provides admission orders for new patients into residential care, including orders for the administration of medications for opioid use disorder. Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care being provided in the setting for both acute and chronic health concerns. Formulates treatment plans based on available clinical data and works cooperatively with patients and care team to implement treatment plans. Conducts on-going clinical management of patients including the management of both chronic and acute illnesses. Supports provision and documentation of the appropriate level of care according to Oregon Administrative Rules applicable to the facility. Determines and manages minor and/or acute illnesses and initiates treatment. Conducts initial care for emergency situations such as illness or injury and initiates arrangements for continuing care. Prescribes medication or other forms of treatment as necessary. Keeps appropriate records. Participates in compliance, quality assurance and quality improvement activities as directed. Is available to take calls from RNs even when off-site when working. This position will provide after-hours phone coverage for Residential Substance Use Disorder treatment programs on scheduled rotation with colleagues. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. Collaborative Care Support pharmacy-based collaborative drug therapy management programs and provide medical visits to support clients receiving treatment. Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs. Collaborates with other health care team personnel and case management personnel. Quality Improvement and Practice-Based Learning Identify opportunities for improvement in medical workflows and logistics. Participate in review and collection of performance and quality indicators; review and contribute to continuous improvement opportunities and plans. Participate in and, at times, lead provider education sessions on a variety of clinical topics, including helping train and onboard other medical providers and staff. Participate in compliance, quality assurance, and quality improvement activities as directed. Collaborate with the care team to ensure optimal performance on team-based performance metrics. Perform other duties as assigned, including work at other CCC clinical sites and/or off-site at community events. Skills and Abilities Ability to consider the impacts and outcomes for underserved communities during decision-making process Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes Must be able to interact with co-workers, patients and staff to instill confidence and facilitate communication. Experience in urgent care and/or primary care practice. Ability to display a non-judgmental attitude. Sufficient manual dexterity and physical ability to perform assigned tasks. Ability to manage time and meet deadlines. Knowledge of de-escalation methods or willingness to be trained in these methods. Benefits at Central City Concern offered to our employees!
