PRN Home Health Licensed Vocational Nurse (LVN) Routine Visits

Viva Life Home Health & Home Care is seeking a dependable and compassionate Licensed Vocational Nurse (LVN) to join our growing home health team on a PRN basis.

This position will primarily focus on routine skilled nursing visits for patients receiving care in their homes. We are looking for an LVN who is clinically confident, organized, comfortable working independently in the field, and committed to providing excellent patient care while maintaining timely communication with the supervising RN and clinical team.

Home health experience is strongly preferred.

Responsibilities

• Perform routine skilled nursing visits in patients' homes under the direction and supervision of the RN.
• Complete focused nursing assessments and monitor patients for changes in condition.
• Obtain and document vital signs and other clinical findings.
• Provide skilled nursing care within the LVN scope of practice and established plan of care.
• Perform services such as wound care, medication-related interventions, injections, catheter care, ostomy care, and disease-specific education as ordered and appropriate.
• Reinforce patient and caregiver education established in the plan of care.
• Promptly report changes in condition, concerns, or abnormal findings to the supervising RN and appropriate members of the care team.
• Communicate effectively with patients, caregivers, physicians, and the interdisciplinary team.
• Complete accurate and timely documentation in the home health EMR.
• Follow agency policies, infection prevention practices, HIPAA requirements, and applicable home health regulations.
• Maintain dependable communication regarding scheduling and completed visits.

Qualifications

• Current Texas LVN license in good standing.
• Home health experience strongly preferred.
• Current CPR/BLS certification.
• Strong nursing assessment and documentation skills.
• Experience with wound care and chronic disease management preferred.
• Comfortable using an electronic medical record (EMR).
• Ability to work independently and manage a flexible field schedule.
• Dependable, organized, and committed to timely documentation and communication.
• Valid driver's license, reliable transportation, and required automobile insurance.
