Key Responsibilities

• Provide skilled physical therapy services to patients within a home health setting across the Anchorage municipality territory
• Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home environment
• Complete OASIS assessments accurately and in accordance with regulatory requirements
• Maintain productivity expectations within a 40-hour work week
• Document all patient care activities and progress in the electronic medical records system in a timely manner

Required Qualifications

• Active Physical Therapist (PT) license valid in the state of Alaska
• Minimum of 1 year of home health experience
• Minimum of 1 year of OASIS experience
• Ability to work a Monday–Friday daytime schedule with no on-call or weekend requirements
• Reliable transportation to travel throughout the Anchorage municipality territory

Preferred Experience / Skills

• Experience with HCHB (Homecare Homebase) electronic medical records system
• Familiarity with home health productivity standards and point-based documentation systems
