Your Key Responsibilities:

• Conduct comprehensive evaluations to assess functional needs, develop individualized treatment plans, deliver evidence-based interventions, and monitor progress to maximize patient independence, safety, and quality of life.
• Deliver evidence-based occupational therapy in accordance with physician orders and the CMS Conditions of Participation (CoPs).
• Ensure timely, accurate documentation and maintain compliance with CMS, payer, and agency standards.
• Collaborate with patients, caregivers, physicians, and the interdisciplinary team to maximize function and independence.
• Educate patients and caregivers on ADLs, home safety, adaptive equipment, fall prevention, and individualized Home Exercise Programs (HEPs) to promote safety, independence, and functional recovery.
• Monitor patient progress, update treatment plans as needed, and communicate changes to the care team.
